How do you put an image on a shirt?

do you have to use an iron-on thing? because once my brother used it, and he got the outline of the paper on the shirt…. and after we put it in the washing machine for a few times… the image faded out in color. I need to make a graphic t shirt… please help ! ):
and please dont send me a custom-shirt designing website (like zazzle.com or something…)
